Staff #2 and Staff #3 did not follow the ?Supervision Policy? on 7/31/25 before they returned to their classroom from the playground with the children. The policy states that ?Accurate attendance must be kept at all times by teachers. All forms of attendance must accurately reflect the children who are present.? On 7/31/25, Child #1, age 3, was left alone on the playground when the rest of the children in his classroom returned to their classroom at 9:42 a.m. Child #1 remained outside, alone, until a second classroom came outside at 10:04 a.m. and teachers realized he was still out there.

Child #1, age 4, was left in the classroom alone and unsupervised for approximately 1-2 minutes.

Evidence: The written agreements between the parents and the center in ten children?s records (Child #1 through Child #10) did not include a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. The Assistant Director acknowledged that the statement was not included in their parental agreements.

8VAC20-780-500-B · Based on observation, the center did not ensure that diapers are disposed in a leakproof or plastic lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches the exterior surface of the storage system during disposal.

Evidence: 1. The trash can located in the Infants room is not foot operated and requires the staff members to use their hands to push the lid in to drop soiled diapers inside the storage system after diapering.
