8VAC20-780-340-A · Based on interviews, photos and documentation review it was determined that the facility failed to ensure the children's care, protection, and guidance. Evidence: 1) in July 2023, there were approximately 22 documented accidents/incidents in the two's classroom including four bites, four scratches, and six bumps/hits. 2) in August 2023 there were approximately 20 documented accidents/incidents in the two's classroom including seven bites and eight scratches/hits. 3) interviews revealed that there were staffing issues and the unusually high number of incidents have been rectified with new/consistent staffing.
