The toddler class was using dried corn kernels in a craft activity and multiple children were putting the kernels into their mouths. The dried corn kernels pose a potential choking hazard.

Areas and equipment outside the center shall be maintained in a clean, safe and operable condition. A long black electrical cord was observed within the fenced playground area, which is directly attached to the exterior of the center. The electrical cord enters the building near the base and extends upward to the roofline. Excess slack from the cord lies loosely along the ground and is draped over the playground fence, making it accessible to children. The unsecured and slack portion of the electrical cord presents tripping, entanglement, and strangulation hazards to children playing on the playground.

On 3/5/2025, Child #1 sustained a head injury and fractured hand while under the supervision of the center. The center was made aware that same day that Child #1 was taken to the hospital and did not submit a serious injury report to the Department.
