    8VAC20-780-340-A · Based on interviews and review of video footage it was determined that staff did not always ensure the care, protection, and guidance of children in their care. Evidence: On 12/19/2022 Child A (age 3 yrs) was left alone in the STEAM Classroom from approximately 3:50 pm - 4:05 pm. During the time child A was in the STEAM classroom they are seen trying to open the door, kicking the door, and at one point got their hand stuck in the door while trying to open the door. Child A was later found by a parent who was entering the facility and returned to administration. Staff in the toddler classroom were not aware that child A was not in their care from the time of 3:50 pm - 4:05 pm.
