Evidence: On September 27, 2021 staff A held her right foot over child 1's head and while the child lay on a cot. The staff used that same foot to roughly move the cot that the child lay on to another position.

On 03/04/2025, according to staff, Child A (13 months old) was fussy, cried continuously, and refused to eat throughout the day. The Daily Sheet indicated that Child A did not eat breakfast at 9:37 am, nor lunch at 11:19am and 2:22pm. A mark was noticed on the child?s forehead by the classroom staff. The parent arrived for pickup at 2:47 pm and was informed about the child?s fussiness. The parent observed injuries on Child A while in the car seat. At 3:29pm, Child A was seen by a physician. The physician noted multiple injuries including scratches, abrasions, and bumps on the child?s chest, abdomen, scalp, neck, ear and forehead. Photographs documented the injuries. The parent stated that the center did not provide a phone call or incident report regarding the child?s behavior and injuries. After interviewing staff, reviewing documents, and making observations, the local department of social services determined the investigation was found for physical abuse.

At approximately 7:09am on 01/21/2025, Child #1 (3-year-old) ran out of his classroom and through two doors located inside the building. Staff #2, a member of management, was sitting in the lobby behind the front desk when they heard a child running and screaming "mommy, mommy." When Staff #2 got up to check what was happening, Child #1 had exited through the front doors of the building. The second door led to the parking lot, where the child?s parent saw him standing on the sidewalk as they got into their vehicle. The parent and Staff #2 met at the center?s front door and brought the child back into the building. Staff #1 was responsible for the care of Child #1, but reported they did not know the child had left the classroom because the child?s parent did not say goodbye when they dropped the child off. Staff #1 was the only staff in the classroom with 11 children, ranging from 2 to 3 years of age.

On 12/11/24, Staff #1 and Staff #2 did not conduct the required movement log process, which includes a face to name check, when children arrived at school and got off the bus at the center. As a result, Child #1 (6-year-old) had fallen asleep and was left behind on the bus for approximately 30 minutes. The center?s procedures for identifying where attending children are at all times states: ?Anytime child(ren) are moving from their designated classroom to another area of the building including but not limited to, City Place, the playground, another classroom, to and from the bus or outside for a walk. The classroom teacher(s) and manager must complete the movement log process.?
