22VAC40-185-340-A · Based on observation, the center did not ensure that when staff are supervising children, they always ensure their care, protection, and guidance. Evidence: On 9/30/21 the Licensing Inspector (LI) entered the playground where the two-year-old class was playing. Both staff members were seated at a table. Two children were sitting next to one another on the ground and one began to throw pebbles into the air. The other child picked up a large handful of pebbles and began attempting to put the pebbles into their mouth. The LI asked the child to stop and directed the teachers over to the children.

8VAC20-780-550-D · Based on interview and review of documentation, the center did not ensure that monthly evacuation drills were practiced as required. Evidence: 1. An evacuation drill was documented on 11/19/21 at 10:00am with 11 staff participating. Timesheets showed that on 11/19/21 at 10:00am there were nine staff present with the director. Seven of the nine staff who were present were interviewed and two staff reported not participating in a drill since around the Summer and two reported that they did not believe they have participated in a monthly drill since they were hired. 2. An evacuation drill was documented on 12/14/21 at 7:40am with 12 staff participating. Timesheets showed that on 12/14/21 at 7:40am there were four staff present with the director. The four staff who were present were interviewed and two reported not participating in a monthly drill since around the Summer.

Video footage reviewed revealed that on 05/27/25 at approximately 3:35pm, three children in the Preparatory Preschool (three-years-old) were out of actual sight and sound supervision of Staff #1 between 10-16 seconds. The staff member was reportedly cleaning up from an art activity on the opposite side of the classroom and the children were positioned by a blind spot created by the bathroom.

The results of the criminal history record information check for State A were obtained on 10/27/23. The results of the criminal history record information check for State B were obtained on 11/27/23. The out-of-state criminal history record information check is required to be obtained prior to employment.
