Evidence: Upon review of digital documentation it was determined that on 04/02/2024 Staff A roughly handles two children during nap time. At 11:35 a.m. staff A (Age 14 months) is seen roughly moving child A by their clothes to position the child onto their stomach. At 11:40 am staff A is seen roughly pushing child B's (Age 14 months) head down twice onto their cot. Staff A is seen repeating the same action at 11:41 am.

    8VAC20-780-400-B · Based on interviews and review of video footage it was determined that behavioral guidance was not always constructive in nature, age and stage appropriate, or intended to redirect children to appropriate behavior. Evidence:

    1.) On 10/27/2023 at 9:45 am the children in the two's classroom were observed sitting on the carpet with Staff A and Staff B and were participating in circle time activities. Child A (age two) was observed attempting to leave the group on the carpet. Staff A is observed grabbing child A by the leg and drags the child back toward their lap. When the child attempts to get up again, Staff A grabs the child by the arm and begins to drag the child back towards their lap. Child A then falls back and hits their head on the ground. Staff A then places child A in their lap. 2.) At 9:47 am during the same circle time activity child B (age two) retrieves a plastic toy horse. Staff B aggressively takes the toy from child B and throws the toy across the room. 3.) At 9:50 am the children in the two year old room begin to retrieve their coats from behind the classroom changing table. While the children are putting on their coats, child B climbs onto the changing table. It is reported that one of the staff yelled loudly in a harsh voice tone at child B. Staff A then proceeds to the changing table to retrieve child B from the changing table. The interaction between staff A and child B is not seen because the changing table was blocking the view of the camera.
