8VAC20-790-190-B · Based on a record review and interview, the vendor did not ensure that each enrolled child's record contains all of the required information to include the name, address and phone number for both custodial parents home and workplace and the name address and phone number of at least one emergency contact.

The record for child #2 and child #4 contained no information regarding the children's fathers' names, the fathers' home address and phone number nor the fathers' place of employment address and phone number or any indication of non-applicability. Child #1's and Child #4's farther are listed on the birth certificate copies in child #2 and child #4's records. The records for child #1 and child #2 did not contain an address for the listed emergency contact to be called in the case of an emergency if the parent's can not be reached.
