    8VAC20-780-340-A · Based on interview, the center failed to ensure care, protection, and guidance when supervising children.

    Evidence: 1. Per interview with adult 1: On 12/27/23 at about 9:45 AM, staff 3 was alone with four infants, one of which was crying while face down on a Boppy pillow on the floor; when asked if she was going to move child 2, staff 3 replied ?Not now."; Adult 1 walked to the office and then walked back to the classroom with staff 4 and staff 5 to find Child 2 was still face down in the Boppy pillow, now asleep. Adult 1 reported about 15 minutes passed between the time child 2 was first observed face down on the Boppy pillow to when they returned to the classroom to find child 2 sleeping face down on the Boppy pillow. 2. Interview with staff 4 confirmed that upon arrival to the classroom, staff 4 witnessed child 2, 5 months old, sleeping on the Boppy pillow face down. 3. The warning label on the Boppy pillow states the following: -Never allow baby to sleep on the Boppy pillow. -Do not allow baby to lie face down on a Boppy pillow.
