Evidence: 1. On April 11, 2022 child # 1 was left on the playground for approximately 5 minutes. Staff # 1 stated that all of the older children (ages 2-5) were lining up at the door and head counts were not completed. Normally they enter one class at a time and on this day a staff person opened the door and all three classes entered at one time. A parent entering the center saw the child on the playground. The child was standing behind a brick wall that was six feet deep and three foot wide that allowed the child to hide without the staff being able to see the child.

8VAC20-780-340-A · Based on interviews with staff, the center failed to ensure when staff are supervising children, they shall always ensure their care, protection and guidance.
Evidence: 1. On April 8, 2022 child # 1 left the building. Child # 1 was wearing noise cancelling head phones and was in the back hallway that was used for a gathering space in the mornings. Child # 1 left the area and passed the staff # 1?s office. Staff # 1 came out of the office while staff # 4 was rounding the corner of the hallway and saw staff # 1 come out of the office. The child went through the first door and continued to the main entrance door. Staff # 1 then went to follow child # 1. A college employee was entering the building as the child was exiting. The child then proceeded to run approximately 200 yards up two flights of outdoor stairs. A few seconds later another college employee/parent was exiting the building after the staff # 1 and child # 1. Staff # 1 was not able to run after the child due to a medical condition. Staff # 1 continued to yell at the child to stop. The child was not able to hear any one because of the head phones. The two college employees caught up to the child prior to the child entering another building and carried the child back to the center.
