    8VAC20-800-560-A · Based on observation of the family day home, children were not always within sight of a caregiver.

    Evidence: 1. Staff #2 placed Child #1 at the feeding table and went upstairs to get Child #2. Child #1 was out of both sight and sound supervision for approximately 1 to 2 minutes. 2. Staff #2 proceeded to diaper Child #2. During this time, Child #1 was out of sight supervision for approximately 5 minutes. 3. After diapering Child #2, Staff #1 placed Child #2 at the feeding table and went upstairs to get Child #3. 4. During this time, Child #1and #2 were out of sight and sound supervision for approximately 1 to 2 minutes. 5. Staff #2 proceeded to diaper Child #3, leaving Child #1 and #2 out of sight supervision for approximately 5 minutes.
