22.1-289.031-B-2 · Appropriate supervision of all children in care, including daily intake and dismissal procedures to ensure safety of children.
1. On 6/1/26, at 9:53 am Staff #1 was observed talking to a parent, at the back entrance door of the center. 2. When the conversation ended in approximately 30 seconds, Staff #1 allowed the licensing inspector (LI) to enter the center after which both Staff #1 and the LI went to the four's classroom. 3. There was no staff member present in the classroom. While Staff #1 was speaking to the parent, she had left 11 children in the classroom alone and unsupervised. 4. Staff #1 stated that another staff member that was working in the room with her had gone to use the restroom. During that time, the parent was at the door and Staff #1 was helping them with dropping off their child.

Staff # 2 stated that Staff # 1 is alone with the children during nap time, while the other staff in the room goes on break. 4. Staff # 2 stated that the CRS for Maryland has not been submitted for Staff # 1. The sex offender search results shave also not been obtained for Staff # 1.

Evidence: 1. The playground used wood chips as the cushioning material under play equipment on the playground. 2. The dirt was visible on the ground under the wood chips underneath the swings. 3. Two climbing structures did not have a minimum of 6 inches of wood chips around the fall zones.
