    8VAC20-780-340-A · REPEAT VIOLATION:

    When staff are supervising children, they must always ensure their care, protection, and guidance. On July 8, 2025, during a field trip, Child A's prescription medication was stored unlocked in a backpack under Staff #1's seat. The backpack was left unattended by Staff #1 at times while they assisted other children and was accessible to children in the group. Staff #3 did not count or verify that all children's medications, including Child A's medication, were returned and placed back into the locked medication storage box at the center upon returning from the field trip. On July 9, when Staff #2 attempted to administer the medication, it was discovered to be missing and was reported lost.
