8VAC20-780-340-A · The center failed to ensure care guidance and protection when Child 1, Child 2 and Child 3, left the center on 7/22/22.
Evidence: Based on the review of records and interviews, the center staff was unaware that Child 1, Child 2 and Child 3, had left the center and failed to have a system in place to ensure that children could not leave the facility. The center failed to have staff at the front of the building which made it possible for Child 1, Child 2 and Child 3, ages 9, 10 and 13 to leave the center and walk down to the nearby Starbucks. Teacher 1 noticed they were missing after 7 minutes and went to the hallway door to notify the person in charge, Teacher 2. At that time Child 1, Child 2 and Child 3, walked back up the hallway after being discovered in the hallway by Teacher 2 as they returned to the center.

8VAC20-780-340-F · Based on the interviews with staff and parents the center failed to keep a child, age 2, in sight and sound supervision.
Evidence: On Friday 8/18/23 at 15:15 a child in the two year old room was left alone in the room for about 2 minutes. Staff 1, counted the children and left the room to go to the large playground at the other end of the center. All 9 children in care were accounted for. according to the child supervision record. When she reached the door to the large playground she did a name to face check and realized one child had stayed behind. She turned around and watched the director and the father enter the classroom. The child was found in the classroom and it appeared that he had gone to the bathroom. The child was not in distress when he was found by the parent and director.

Evidence: An inspection of the physical plant and the alarm the center has installed as part of the Intensive plan of correction was conducted. The alarm the maintenance department has installed is not sufficient. The alarm sounds like a door bell and rings one time when opened. The sound can not be heard in the center, only at the front desk and office. At the time of the inspection no staff were present in the office and at the front desk.
