8VAC20-780-340-A · When staff are supervising children, they shall always ensure their care, protection, and guidance.
1. On 5/8/26, center staff were unable to ensure Child #1's care, protection and guidance for approximately 4 minutes as Child #1 had walked out of the building's front door. 2. At 4:35 pm, Child #1 was using the one of two restrooms in the hallway and was under Staff #1's supervision. Staff #1 was standing at the corner of the hallway where the restrooms are located. 3. Staff #1 stated that over the next 3-4 minutes, she had turned her face away from the restroom as parents were leaving and saying goodbye. 4. Within these few minutes, Child #1 left the bathroom and walked out with the group of parents and children that were leaving the building, walked down the stairs leading to the main entrance of the center and stood on the grassy area at the bottom of the stairs. 5. Staff #2 who was also supervising the group of children with Staff #1 had stepped into the first room across from the restroom and saw Child #1through the window, standing outside on the grassy area. Staff #2 alerted Staff #1. 6. Child #1 was brought back in at 4:42pm by Staff # 1.
