8VAC20-780-270-A · Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: 1) observed that there were two access boxes with water valves accessible to children in the main preschool room, and the small "green" room. The main preschool box had tape keeping it shut, and the green room box would not close. 2) observed that there were 2 nap mats with tears in them. One nap mat had tears along the seam, and the other nap mat had tears along the body of the mat, exposing the absorbent material beneath.
