    Evidence: On 11/7/23 at approximately at 9m during circle time, staff A can be seen on video footage sternly telling child A to put their shoes back on after the toddler had taken them off. The voice tone was harsh and not intended to constructively redirect the toddler.

    8VAC20-780-330-B · Based on observation, it was determined that the playground did not maintain an adequate resilient surfacing around playground equipment and did not maintain adequate fall zones.

    Evidence: 1. On the older playground the Licensing Inspector observed divots in the poured-in-place resilient surfacing. One divot was located 2 feet in front of the end of the single tan slide with the other divot located 4 feet in front of the end of the double tan slides. 2. On the younger playground the Licensing Inspector observed a portable blue and green slide within 4 feet of the Awning pole on 3 sides and within 5 feet of the chain link fence on the 4th side. The standard requires this slide to have a 6 foot clearance from all obstacles.
