Evidence: There was no orientation training on file for staff # 1 ( hired 8/21/23) and staff #2 ( hired 7/11/23). The orientation provided to staff # 3 ( hired 6/20/23), #4 ( hired 7/14/23), #5 ( hired 6/23/22) and #6 ( hired 6/23/22) were missing the following components: * emergency preparedness and response planning such as violence at a child day facility ; * Use of safe sleep practices; * Prevention of abusive head trauma, including procedures to cope with distraught children; *Prevention and response to emergencies due to food and other allergic reactions; and * Prevention and control of disease.

The center has failed to obtain the out of state criminal history search for California prior to the first day of employment. The center also failed to obtain the out of state child abuse and neglect search request by the end of the 30th day of employment.
