Evidence: On 1/11/2022 at approximately 4:15 pm, there was not a staff member present that was trained in daily health observation within the past three years. Staff #1 was present and the training was last completed on 6/4/2018.

The record for Child #3, enrolled for six months, was missing the address of two designated people to call in an emergency if a parent cannot be reached.

8VAC20-780-130-A · The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #2 attended the center for one week before submitting documentation of an immunization for the child?s record.

8VAC20-780-140-A · Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #5, who had been enrolled for over two months.

Evidence: The following was found during review of injury reports: 1. The record for child #2 had documentation of injury sustained on 10/2/2023. The parent signed the form on 10/5/2024. Site Director stated the parent was notified on the day of incident but that date was not documented. 2. The record for child #4 had documentation of injury sustained on 9/29/2023 and the time of parent notification was not documented. 3. The record for child #5 had documentation of injury sustained on 9/20/2023 and the time of parent notification was not documented. 4. Site Director confirmed missing information.

Evidence: The following was found during record review: 1. The record for child #2 (start date 12/5/2022) did not have emergency contact addresses and physician information (name and phone number). 2. The record for child #3 (start date 9/7/2022) did not have a second emergency contact, physician information (name and phone number) and allergy action plan for a food allergy noted on the child's physical examination.
