Evidence: On 01/25/2024 Staff 2 was notified that upon arriving at home from care that day, the parent of Child 1 (age 9 months) discovered that Child 1?s Velcro bib had been tied tightly in a double knot around their neck. Pictures of Child 1 show that the bib was double-knotted and tied tight enough to leave a visible red mark around the child?s neck. It was determined through staff interviews that Staff 1 had tied the bib around Child 1?s neck. Staff 1 did not ensure the care and protection of Child 1 when she tied the bib tightly around the child?s neck.

Based on video footage and staff interviews, the center failed to ensure that when staff are supervising children, they shall always ensure their care, protection and guidance. Evidence: Video footage from 11/29/2023 shows Staff 1 putting 5 children (ages 3 years and 4 years) into the backseat of a mid-sized car. Based on Staff 1 statements, the same 5 children were transported to the center for afterschool care on 11/28/2023 and 11/29/2023 in the mid-sized car that was their personal vehicle. Staff 1 stated that one child got into a car seat that was in the backseat of the car and the other four children were also in the back seat buckled into the same seatbelt and not in a child restraint device. The children were transported approximately 3 miles. The Code of Virginia requires that any child, up to age eight, shall be properly secured in a child restraint device of a type which meets the standards adopted by the United States Department of Transportation. The center did not ensure the care, protection and guidance for the children during their transportation to the center when four preschool age children were transported in a mid-size care without the required child restraint devices.

Staff 1 stated that the pill box belonged to Staff 2. The staff persons medication was not kept in a locked place to prevent access by children.
