The center?s required playground safety procedures state that teachers should be circulating and interacting with the children at all times. 04/23/2026, Class 1 with nine children (ages 16 months-24 months) were outside on the playground for approximately 50 minutes. The two staff supervising the children sat in stationary locations, not circulating the playground or interacting with the children while on the playground.

Staff 1 stated that Child 2 was outside of their sight supervision at that time.
