Evidence: The center's Face to Name policy for accounting for children at all times requires staff to compare the children's faces to the name listed on the written attendance during transitions from different areas of the facility. On September 21, 2023, Staff A, staff B, staff C did not complete a name to face check of all the children after transitioning from the outside play area to the classroom. Child A was left unsupervised on the outside play area for approximately three minute as the class entered the building. On October 26, 2023, Staff A and staff D did not complete a name to face after two children transitioned from the classroom after diapering and using the restroom. Child B was left unsupervised in the classroom bathroom for approximately 5 minutes.
