On December 16, 2025, Staff #1 grabbed Child #2 (age 4 years) by one arm, suspending the child in the air for approximately two seconds and moved the child two feet away on the carpet.

8VAC20-780-410-5 · Based on staff interviews and review of documentation, the center failed to ensure that staff did not engage in forbidden action of forcing food when supervising children. Evidence: On June 19, 2024, Staff #2 forced open child #3?s, age 3 years old, mouth and shoved a piece of bologna found on the lunch table into Child #3?s mouth. Staff #2 then proceeded to move child #3?s jaw in a chewing motion.

8VAC20-780-510-L · Based on observation and interview, the center failed to ensure medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence: In classroom 9 (ages 1yr-3yr), there was medication in an unlocked bag the staff was wearing on her back. The medication bag had the key inserted in the keyed lock. The medication bag was broken and couldn't be locked. Administration confirmed the bag was broken and unlocked.

Evidence: On June 11, 2024: 1. Staff #1 roughly handled Child #2, age 2.5 years, by pushing Child #2 in the forehead with Staff #1?s hand. Child #2 cried and pointed at Staff #2. Child #3 told Staff #3 ?teacher pushed forehead? 2. Staff #1 roughly handled Child #2, age 2.5 years, by grabbing Child #2 by the wrist and jerking Child #2?s wrist back. Child #2 cried.

Evidence: 1. On 07/24/2023, Staff #4 reported that the parent of Child #1 had received a video of Staff #1 hitting Child #1 via social media (Snapchat). Staff #4 noted that the video had been observed and confirmed that the video was taken at the center. 2. During an interview on 07/27/2023, Staff #3 confirmed that they had taken the video at the center on 07/03/2023 and that they had sent the video to a friend via social media (Snapchat). 3. Review of the video by the Licensing Inspector on 07/25/2023 confirmed that Staff #1 swatted Child #1 and had also pushed Child #1 to the ground. 3. Page 13 of the center?s policy regarding confidentiality in the ?Staff Orientation Handbook? states: ?No information or pictures of the program and the families and the children we serve shall be posted on the internet/social networking such as Facebook or Twitter. No pictures taken unless given permission by Staff Mangers.? and page 9 states: ?Unacceptable: a. ANY form of physical punishment!?.
