Based on a review of seven staff records, the center did not obtain the results of a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which one staff member had resided in the preceding five years. Evidence: 1. The record of staff #3, employed 06/05/2023, indicated the staff resided outside the state of Virginia in the past five years. The record did not contain documentation of an out of state child abuse and neglect search request and a sex offender registry check for the state. 2. A member of management stated the out of state background checks had not been requested.

8VAC20-790-600-D-1 · Based on review of four staff records, the center did not ensure all staff who work directly with children obtain current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care within 90 days of employment.

Evidence: 1. The record of staff #1, employed 11/21/2022, did not contain documentation of CPR certification. The director stated staff #1 did not have CPR certification. 2. The record of staff #2 and staff #3, both contained documentation of CPR certification that expired 04/02/2021. The director stated the two staff members had not renewed their certification in CPR.

The director confirmed a request to update the fingerprint national criminal record check and central registry check for staff #4 had not yet been obtained.
