Staff #1 & 2 have a CPR certification that was obtained online and is not from an approved provider.

Evidence: The violation notice dated 9/25/23 was not posted.

8VAC20-800-210-A · Based on record reviews the provider failed to ensure that caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence: Staff #2 has not obtained training hours for this year.

8VAC20-800-920 · Based on observation and interviews the provider failed to ensure that when meals or snacks are provided by the family day home, the menu for the current one-week period shall be posted given to parents or posted in an area accessible to parents.

Evidence: The provider stated that she provides some meals and snacks for the children in care. There are no menus posted or provided to parents.

Evidence: Located on the bed, in the first bedroom in the hallway, were two prescription bottles and a bag of Halls cough drops. The door was slightly opened. The child in care was mobile and had access to this area.

Evidence: The provider and assistant have a CPR certificate that was conducted on-line. There was no in-person component involved.

8VAC20-800-100-A · Based on review of children's records the provider failed to obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

Evidence: Child #2 (first day 2/15/22), child #3 (first day 2/16/22), and child #4 (first day 2/16/22) do not have a physical on file.
