Evidence: On the large playground, there was grass growing through out the use zones of the play equipment. Mulch varied between 1-4 inches around all of the equipment including but not limited to the swings, rockers, the climbing wall, slides and under the rungs.

Evidence: 1. On both climbers at the top of the slides, there have been pieces of composite decking boards (approximately 4inches by 8 inches) installed to close a head entrapment point. The gaps between the slide and the panels and between the pole and the panels present an entanglement point. 2. The bolts used to install these panels have more than 2 threads of the bolt showing presenting an entanglement point.
