All six swings had rusting chains and the merry-go-round had rust on all seven handlebars, gripped by children on the playground. Child A's infant crib had a two inch dried stain that had not been cleaned in daily sanitation routines in addition to a three inch loose plastic label. The infant room's diaper changing table had areas of peeling paint measuring about three inches. The Toddler classroom's floor mat had two torn and loose sections measuring one inch. The Toddler classroom's wooden bookshelf had three shelves with chipped and peeling paint, affecting an area of approximately three feet.
