    22.1-289.031-B-2 · Based on observation and an interview, the center did not ensure to implement the center?s established supervision procedures.

    Evidence: 1. The center?s administrator stated that the supervision procedures established for the center are to ensure that all children are always within sight and sound supervision. 2. Upon entering the two year old classroom, there were 13 children within sight and sound supervision. 3. A two year old teacher stated that there were 14 children in care. 4. Approximately 3-5 minutes after the inspector entered the two year old classroom it was discovered that a two year old child was playing in the open closet in the classroom, out of sight supervision.
