    Based on interviews, the center failed to ensure that when staff are supervising children, they shall always ensure their care, protection, and guidance. Evidence: Interviews with staff #1, staff #2, staff #3, and staff #4 revealed that on July 28th child #1 (4 years) followed staff #1 out of the classroom onto the playground. Staff #1 was unaware that child #1 followed them through the exit door and onto the playground. Staff #1 was signed into the classroom tracking sheet (CSR) and had responsibility for the children in classroom #1. Child #1 was left unsupervised on the playground for approximately 5 minutes. The temperature on July 28th was 98 degrees. There was a heat advisory in place. The lawn crew was on the playground cutting the grass. The child was found crying and had visible signs of being overheated.
