    Evidence: 1. The record for child #1, date of enrollment 08/21/23, stated that the child has several food allergies and intolerances. 2. Written correspondence revealed that child #1 was taken to the doctor and was advised to avoid certain foods due to an intolerance. 3. Documentation review and interviews revealed that child #1 was offered foods and allowed to eat foods that were listed in the child's record as a food allergy or intolerance, as well a direct written request from the parent to avoid those foods, which caused the child to have an upset stomach, vomiting, and gastrointestinal upset.
