    Based on documentation and interview, the center did not ensure compliance with ratios of staff to children as required. Evidence: (Repeat Violation) On 03/27/2024, the licensing inspector entered the Infant I room at approximately 8:39 a.m. and reviewed the ?Face-To-Name Transition Sheet?. The Face-To-Name Transition Sheet documented S1 being in the classroom by herself between 7:22 a.m. and 7:43 a.m. with 10 children until S2 removed 4 children from the classroom at 7:43 a.m. The Face-To-Name Transition Sheet documented S1 being in the classroom alone again between 7:43 a.m. and 8:00 a.m. with between 6 and 11 children when S3 removed 7 children from the classroom. S1 acknowledged the room was out of ratio several times that morning as well as other times on other days. S4 reviewed the Face-To-Name Transition Sheet and acknowledged the Infant I room was out of compliance with the allowed ratio of 1 staff to 5 children due to the ratio waiver being granted.

    On 2/13/2024, the licensing inspector entered the Infant I room at approximately 8:42 a.m. and observed 8 infants being cared for by 1 staff member (S1). The infant classroom is allowed a ratio of 1 staff to 5 children due to the ratio waiver being granted. S1 acknowledged the room was out of ratio when licensing inspector entered the room as well as other times on other days. The ?Face-To-Name Transition Sheet? documented S1 being in the classroom by herself between 8:14 a.m. and 8:44 with 7 children. A second staff (S3) entered the classroom at approximately 8:44 a.m. On 2/13/2024, the licensing inspector entered the Infant 2 room at approximately 8:47 a.m. and observed 8 children being cared for by 1 staff member (S2). The infant classroom is allowed a ratio of 1 staff to 5 children due to the ratio waiver being granted. S2 acknowledged the room was out of ratio at that time as well as other times on other days. The ?Face-To-Name Transition Sheet? documented S2 being in the classroom by herself between 8:16 a.m. and 8:57 a.m. with eight children. S3 entered the classroom at approximately 8:57 a.m. and removed five children from the classroom. The S4 was interviewed and reported that the center had multiple staff members call out due to illness the morning of 02/13/2024.
