8VAC20-790-560-A · Based on record review and interview, it was determined that the vendor did not ensure that staff be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB) within the last 30 calendar days of the date of employment.

Evidence: 1. Staff #2 (date of hire 9/07/21) had her TB screening completed on 12/07/21 which is three months after their hire date. 2. Staff #3 (date of hire 9/01/21) had her TB screening completed on 3/21/22 which is 6 months after their hire date. 3. Staff #1 confirmed that both Staff #2 and Staff #3 did not have their TB screening within the last 30 days of the date of employment.
