Staff 1 stated that she checked the back of the child and noted it was "a little red" and offered ice which the child declined. b. No injury report was written for the child who was kicked in the back by child 1. 2. A review of written incident reports indicates that child 1 "punched another child in the chest" on 07/11/2022. A second incident report indicates that child 1"attacked a child physically" on 07/11/2022. a. No injury report was written for the child who was punched or the child who was attacked physically. 3. Interviews with staff 1, staff 1, staff 2 and staff 3 confirmed that injury reports had not been written for the incidents occurring on 07/11/2022 and 08/03/2022. a. Administrative staff confirmed that injury reports should have been written for these incidents.

8VAC20-780-270-A · Based on observation, the center failed to ensure that areas of the center, inside and outside, shall be maintained in a clean and safe condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence: 1. Numerous exposed tree roots were observed throughout the ground of the the outside area of the facility utilized by staff and children to enter/exit to the playground from the three's and four year old classrooms. 2. An area of broken and cracked wood was observed on the bottom area of the facility outside of the three-year old classroom doorway to the playground. 3. Areas of peeling paint were observed on the lower area of the wall behind the toilet in one of the kindergarten bathrooms.
