8VAC20-780-370-1 · When an infant is placed in his crib, he shall be placed on his back. On 9/9/2024 Staff #1 placed Child #2 in a crib, flipped him onto his stomach and walked away from the crib. Later, another unidentified child was placed in a crib on their stomach. Staff #1 proceeded to pat the child?s back.

Resting or sleeping infants shall be individually checked every 15-20 minutes. On 9/9/2024 no staff checked on sleeping Child #1 for over two hours.

On August 30th Staff #1 and Staff #2 did not follow center supervision policies which requires completing face to name checks when transitioning children from the playground back into the building and classroom. As a result of the policy not being followed Child #1 (30months old) was left unsupervised on the playground for approximately 2 ? minutes.

The chemicals were in the lower storage cabinet of the diaper changing table and accessible to children. b. There were 10 children in care at the time of the inspection. 2. The program director confirmed that the bleach and sanitizer were left unlocked and were accessible to the children in care.
