    Evidence: 1. The record for child #1 did not contain proof of identity. 2. The record for child #2 did not contain the medical insurance policy number and the address for one of the two required emergency contacts. 3. The record for child #5 did not contain: - Child's full name, nickname (if any), sex, address, and birth date; - Emergency contact information including - Name, address and telephone number of each custodial parent's place of employment; - Name, office address and telephone number of the child's physician; - Name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached; - Information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation; - Name and policy number of the child's medical insurance, if applicable; - Names of persons other than the custodial parents who are authorized to pick up the child; - Appropriate legal paperwork when a custodial parent does not authorize the provider to release the child to the other parent; and - Chronic physical problems, pertinent developmental information, and any special accommodations needed 4. The provider confirmed that this information was not available during this inspection.
