Evidence: 1. The administrator stated the center's policy for immunizations is as follows: Upon enrollment or before the first school day parents need to submit the most up to date immunization records of their child. If somebody does not have an immunization record then we ask them to submit the Religious Exemption Form. 2. The record for child 2 was reviewed. The first date of attendance was documented as 1/31/22. The immunization record was dated 2/25/22. 3. The administrator stated "The parents never gave them the copy when registering. I reminded them a few times but then we were on quarantine for 10 days and got back to school on February 23." She brought it on the 25th.
