This list shall be dated and kept confidential in each room or area where children are present.

The allergy list in two rooms was dated February 2025 and the allergy list in one room was dated January 2025. Each list contained information regarding a child that is no longer at the center. One allergy list was hung on the front of a cabinet in the classroom.

8VAC20-780-440-E · There shall be at least 12 inches of space between occupied cots, beds, and rest mats. Exception: Twelve inches of space are not required where cots, beds, or rest mats are located adjacent to a wall or a divider as long as one side is open at all times to allow for passage.

Staff #1 and Staff #2 did not follow procedures in completing the Child Supervision Record (CSR) by checking. A review of CSR document dated November 8, 2024 showed staff not checking out.
