Evidence: 1. The pink tunnel on the play structure on the playground has a large crack in the middle. The crack spreads off into different directions, and creates sharp edges and pinch points. 2. The playground mat at the end of the slide is buckled in the middle and cracked. The buckling of the mat creates a trip hazard at the end of the chute of the slide.
