The center did not follow their own written procedures for safe sleeping practices which state the following: No loose bedding, pillows, bumper pads, etc. will be used in cribs. In the Angelfish classroom a loose blanket was in the crib with a five-month-old sleeping baby.

Evidence: The record for child 1 was reviewed. Per interview with staff, child 1 has a food allergy and the allergy was posted in the classroom and cafeteria. There was not an allergy care plan in the file. The director stated they did not obtain an allergy care plan for child 1.
