On the morning of 4/8/2024 Staff 1 was in the Two year old classroom supervising 10 children. The maximum allowable child to staff ratio for a two year old grouping is 8 children to 1 staff member. 2. At 9:10 am on 4/8/2024 Child 1 (age 2) fell, hitting their head on a bookcase. 3. Staff 1 did not have the required CPR and First Aid training to be supervising children alone. 4. Child 1 required 6 stitches to their forehead for the sustained injury.

On the morning of January 18, 2024, while the toddler room was out of ratio, Child 1 fell and hit their mouth on the, requiring first aid. 3.
