Evidence - 1. On the date of inspection (05/13/2024), it was determined that Staff #1 started as a volunteer on 07/28/2022, and then became a part-time staff (floater) as of 10/01/2022. 2. From 10/01/2022 until the FPC was discovered (through self audit) to not to have been done, such documentation was not on site. Staff #1 worked and was paid during this time. 3. A FPC was sent for and was then completed for Staff #1 on 05/07/2024.

On the date of inspection (01/04/2023), it was determined that the following staff did not have a current certificate of completion on file for the 2023 Annual Health & Safety Update Training: Staff #4 (the last certificate on file was dated - 03/25/22), Staff #5 (the last certificate on file was dated - 03/25/22), Staff #6 (the last certificate on file was dated - 07/20/22), Staff #10 (the last certificate on file was dated - 09/15/22), Staff #11 (the last certificate on file was dated - 01/23/22), and Staff #13 (the last certificate on file was dated - 03/17/22). 2. This finding was confirmed on the date of inspection by the Center Director.

Evidence: Child #6's record does not contain documentation of a written care plan for a child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
