1.Staff #1 confirmed that on 03/20/2025, Child #1 (age 26 months ) was served a known allergen at lunch time, exposing the child to an allergen. Staff #1 ( in training) did not verify Child #1?s identity before serving child # 1 2. Child #1 showed symptoms of an allergic reaction and had to be administered an over the counter antihistamine and an emergency medication ( per parents request). The center called 911 and the child was transported to the hospital.

Child #1 suffered an allergic reaction and had to be administered Zyrtec and epinephrine injection then 911 was called and child was transported to the hospital.
