In two classroom bathrooms, the vinyl flooring was peeling behind the toilets. In one classroom bathroom, empty plastic grocery bags were hanging above the toilets within reach of children.

On the large playground, the asphalt sidewalk had 3 pot holes in it. One pot hole was approximately 6in. x 6in. , the second pot hole was approximately 6in. x 4in. , the third pot hole was approximately 1ft. x 3in. 3. On the blacktop, one bike was missing 1 foot pedal. 4. Staff #1 acknowledged the holes, pot holes and missing foot pedal during the inspection.

VA-ECC card.? 2) Witness #1 confirmed that the center has possession of parents VA-ECC cards. 3) Witness #1 stated that the VA-ECC cards are kept in a room that the parents do not have access to. 4) Staff #1 stated that Staff #2 has been in possession of VA-ECC cards. 5) Staff #1 stated that they have seen Staff #2 help parents swipe the VA-ECC cards. 6) Staff #2 confirmed that the center has had possession of 2 parents VA-ECC cards. 7) Staff #2 confirmed that the center no longer has possession of any parents' VA-ECC cards.

Child #1 (age: 5 years) hits Staff #1. Staff #1 yells ?slap me again, slap me again.? At approximately 4:53pm, Staff #1 yells ?are you going to hit me, hit me in my face again.? Child #1 hits Staff #1 in the face, then Staff #1 hits the child on the cheek.

Staff #2 arrived to the classroom at approximately 8:41 a.m. and left with two children, leaving Staff #1 with 10 children, within ratio. 3. Video footage shows 3 additional children getting dropped off in the classroom between approximately 8:50 a.m. and 8:57 a.m. leaving Staff #1 with 13 children, exceeding the 10:1 ratio, until Staff #3 arrived at approximately 9:08 a.m.
