    Based on observation of the Pre-K classroom, staff were supervising children and did always ensure their care, protection, and guidance. Evidence: 1. The two assigned staff members in the classroom stated 18 children were in the classroom. Inspector was in the classroom approximately 3 to 4 minutes observing the classroom and there were 20 children. When it was time for the children to transition outside, staff member 1 counted 20 children touching each child's head.

Based on observation, a changing pad had visible tears. Evidence: 1. Director will discuss with staff to follow cleaning checklist. Closing staff will help assist with bathroom cleaning. A work order has been sent to maintenance for repairs. 2.Changing pad was observed with tears in the waterproof cover.
