maintained in compliance with the center's own policies and procedures. The center?s policy/protocol regarding forbidden actions was not followed. On 11/6/2025, staff #1 was observed and stated they called child#1 a name and subjected children to profane language.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. The written classroom attendance form in classroom #3 was not accurately marked for children in attendance. The form had 12 children and there were 16 children in the classroom.
