school program, sight supervision of 1 child was not maintained. Evidence: 1. The after school class was outside on the playground and blacktop area. 2. Staff #1 was assisting children with selecting equipment from the storage building while Staff #2 took one child inside to wash hands. Staff #1 was observed going inside the storage building with one child to select equipment. 3. During this time, one child left the blacktop area and went into the fenced playground area and climbed up onto a piece of equipment and proceeded to climb across the monkey bars. 4. Staff #1 and #2 was unaware that this child was on the playground area.
