    8VAC20-780-340-A · When staff are supervising children, they shall always ensure their care, protection, and guidance.

    On 6/11/26 two, young, 2 year old children were sitting on a toddler/preschool height table that they had climbed up on. Staff #1-#3 were engaged with other children in various tasks and gave verbal commands for the children to get down. Both children fell off of the table, child #1 hit their head resulting in a goose-egg on the forehead. Child #2 did not have any visible marks. Staff #1-#3 did not ensure the care, protection, and guidance of the children as they did not go over and assist the children with safely getting off of the table. Video showed the length of time the children were seen on the table was 2 minutes, before the children fell.
