8VAC20-780-400-B · Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
On 06/23/26, within a 10-minute window, staff #1 took child #1 (4 years old) into the bathroom several times and held the child's shoulders and arms from behind while splashing water into the child's mouth and face because the child was using "potty" words. The child was heard crying, screaming "no" and "get off of me", during the incidents. The response from staff #1 was not constructive or age and stage appropriate.

On 11/13/25 staff #1 did not provide constructive behavioral guidance to child #1 when trying to intervene during an incident between child #1 and another child. Staff #1 grabbed child #1 by the back of his overalls lifting him slightly and moving him away from the incident.

children in decision making. Evidence: Staff #1 was seen on video nudging child #1 in the back 2 separate times on 9-14-22 while trying to get her to proceed to the potty after her diaper was removed. The nudges did not appear to be forceful, and the child did not react to the nudges. Staff #1 did not assist the child adequately in decision making skills during this incident.
