Evidence: Verified through written statements and interviews with staff, child # 1 was out of sight and sound supervision for approximately 2 to 3 minutes. The children were lined up to enter the building from the outdoor play area. Child # 1 wondered from the entrance to the playground to the front of the building. Once realized child # 1 was missing, staff # 2 went out the front door and staff # 1 went around the back. The child was located in the front of the building around the shrubs.
