The facility did not comply with their mediation policies requiring prescription medication to be kept in a locked location and over the counter ointments to be inaccessible to children. There was prescription lotion and topical ointment for child 5 stored in the child's open low cubby. There was no written parent permission for the application of the prescription lotion.

Evidence: There was water-filled bottle warmer on a shelving unit beside the diaper changing table. the cord from the bottle warmer was hanging loose between the shelf and the changing table, allowing for the possibility that a child could pull the bottle warmer off of the shelf onto him/herself.
